New Patient Registration


If you live in our practice area and wish to register as a patient at our surgery then we need certain information from you, and each member of your family, so as to be able to trace your previous medical records. We advise you here on how you may register as there are several methods of doing so

  1. You may call into the surgery and complete a registration form for each family member. You will need to provide proof of identity - passport, driving licence photo ID or birth certificate and proof of address
  2. You may print off and complete the registration questionnaire HERE. You will need to do this for each family member and then drop it into the surgery.


Note that we cannot guarantee that we shall accept you as a patient since, for example, it may be that your address is not in our catchment area. If you register online and we send forms to you for signature then you will know that we are able to accept you, or if you attend the reception for registration in person the receptionist will be able to advise you.


As a new patient, on regsitration, we invite you to have a health check with one of our health care assistants. You will also be informed of who you will be registered with as your  Named Accountable GP, however you are welcome to see any GP of your choice and can request to change your named GP at any time.
